Corrective Action Effectiveness Verification

Issuing a corrective action is not the end of the process. Effectiveness verification confirms that the root cause was genuinely eliminated and the problem has not recurred.

Effectiveness check methods

MethodDescription
Recurrence monitoringTrack the original failure mode for 30/60/90 days; zero recurrences = effective
Process auditAudit the specific step where the root cause was found
Data comparisonCompare metrics before and after (PPM, Cpk, defect rate)
Similar part checkLook for the same root cause in comparable parts or processes

Effectiveness rating scale

Some quality systems use a 1–5 scale: 5 = root cause eliminated and system prevents recurrence; 1 = problem recurred unchanged. The rating drives whether the CAPA record can be closed.
